Grinder Wheel Exploded/ Grinder Safety...(?)

Hello All,
I've been trying to hold off on new posts until I can post cool pics.
But I feel I NEED to mention this:
I have a bottom line Ryobi angle grinder.
I got it for $39 IIRC, and they're $29 now.
Great buy IMO.
It spins 11,000 RPM with a 4.5" disc. (Probably the "no-load-speed" But still it's fast)...
I used Bosch angle grinders at my old steel factory job that had massive lay-offs in 2009.
It was my FAVORITE tool there to use grinding the welds smooth ,etc.
Here's my point:
I was grinding a rusty steel wing-nut off of a cast aluminum BBQ grill top.
It was less than 1/4" inch bolt.
Cut the first off, No Problem.
The second cut , For NO REASON (that I can imagine),
THE DISC EXPLODED!!! :
One piece shot forward, and another chunk whacked my shoe (foot) SO hard , Instantly I thought IF it had hit my glasses , it would have penetrated them easily!
Wow , it's still my favorite tool.
I've spent probably hours using angle-grinders and this was the first incident such,
But anyway I want to wear probably a full-face sheild from NOW ON!.
There's the issue with grinding wheels too, where IF they are wet, the liquid will go to the unbalanced portion of the grinder,
and become so "lop-sided" the wheel will fly apart and can cause DEATH on impact.
But this was a dry , normal disc.
I just don't understand it.()
Doesn't matter, I will probably wear a full face sheild in the future.
It seems even when I used to wear my prescription glasses covered by safety glasses ,hot sparks always found a way around BOTH somehow.
Nothing compared to this explosion the other day though.
Oh, also one time I bought a rod-coupler nut (it's used to connect two threaded rods together, looks like a giantly wide nut)
It fit the Ryobi thread so I put washers and layers of denim rags.
...Thought it would make an awesome polishing wheel, but when I turned it on: instantly the denim "exploded" in every direction...made me laugh. So Still NOTHING like this cut-off disc exploding the other day.
Just felt like sharing this since even with personal protection gear, consideration and MORE is needed for angle grinding..
I DO feel lucky too.

EDIT: forgot to mention, One time in the factory, I was grinding away on a weld and the sparks lit some oily rags beneath my feet on fire! That one was funny too because I'm standing inside on concrete and a fire blazed up ,and I was frantically stomping it out.
