I'd imagine he was holding it like one holds a katana with the thumb and forefinger loosely circumscribing the tsuka , gripping with the pinky and ring finger , but not real tightly... Not necessarily just not holding the tsuka tight enough. But if you pause the video at 0:14 you can see that he has a WAY wrong grip to perform Nukitsuke in that manner. Without the proper grip the sword will just torque right out of your hand.

this reminds me of the video of the guy on TV trying to sell katanas say how tough these ones are while smacking it onto a table. The blade breaks and stabs him in the shoulder.... I saw that one too...was selling those cheap Pakistani straight edged 'katanas'; I actually had one that broke at the handle, first sword I ever bought. I still have it...as a reminder that you get what you pay for. I paid $12. I guess you could say everyone makes mistakes, even elementary ones like letting your sword fly from your grasp- but to have it for posterity has to really, really suck. Especially if this were an isolated incident...thing is, he didn't appear to be all that surprised when it happened, which leads me to believe he may have done that a time or two before. Thanks for sharing.

Single handed cut has different physics to a two handed cut, you need a tighter grip on the tsuka with a single handed cut, single handed cuts are best made with a modified hammer grip.
