Closed cell foam (e.g., Ensolite). For three reasons: (1) It works. (2) It's easy to adjust the padding to fit my head. Where there is too much space between head and padding, just glue in another layer. (3) It's explicitly SCA-legal (if thick enough).
It's also cheap and easy. Not historically accurate. Ugly, too (but not very visible when worn).

The problem is lack of ventilation, with the padding cutting off airflow. I was thinking that padding a helmet with vertical strips of padding, with gaps between them, might be good. Have't tried it yet.
Don't worry about filling the empty space in the top. Better to have empty space there (see "ventilation" above; empty space and a hole in the top might be good). The padding is there to keep steel from your skull, by progressively stopping the helmet from moving to smack your skull.

I've heard that suspension liners work well when you're also wearing a padded coif, haven't tried it though so I couldn't say for sure. Leaving gaps in the foam padding for ventilation is a good idea. You also want to put a fabric liner in over the foam, or wear a coif, to keep the foam off your head. It makes it much nicer.
